MDD Maritime Declaration of Dealth Details

The MDD notification provides person-specific health information that supplements the MDH notification (Maritime Declaration of Health) for a vessel calling at a Finnish port. The notification provides more detailed information on, for example, ill persons, symptoms, medical treatment, evacuations, and other health-related events of significance.

The MDD notification supplements the MDH notification and is submitted together with the MDH notification.

Data to be completed

Remark DE-003-01

Complete this field if you wish to provide additional information related to the notification that cannot be reported elsewhere. For example, you may provide further details concerning the health situation on board, sanitation measures, or the processing of the notification.

If there is no additional information to report, leave the field blank. Do not repeat information already provided in other fields.

Person on board

Provide information in this section about a person who is ill, deceased, evacuated, or otherwise involved in a health-related event of significance. Each person must be reported as a separate entry.

Crew member rank or rating DE-035-14

Select the crew member's rank or qualification (STCW qualification) on board.

Examples of available values:

  • 10100 – Master
  • 10110 – Chief Officer / First Officer
  • 10150 – Chief Engineer
  • 10210 – First Engineer Officer
  • 10330 – Electro-Technical Officer (ETO)
  • 10400 – Boatswain
  • 10420 – Able Seafarer Deck (AB Deck)
  • 10450 – Ordinary Seafarer Deck (OS Deck)

If the crew member does not hold an STCW certificate of competency (for example, crew of a recreational vessel), select the closest matching general title, such as 10460 – Deckhand / General Deck Crew (without qualification).

Person type DE-035-01

Select the type of person.

Available values:

  • FM – Crew member
  • FL – Passenger
  • DEE – Stowaway

If you have completed Crew member rank or qualification DE-035-14, select FM – Crew member.

Person gender DE-035-08

Select the person's gender as indicated in the travel document.

Available values:

  • 0 – Unknown
  • 1 – Male
  • 2 – Female
  • 9 – Not applicable

Use value 0 – Unknown only if the gender is not indicated in the person's travel document.

Use value 9 – Not applicable when the gender field in the travel document contains X or another designation other than male or female.

Person given name DE-035-03

Enter the person's given name exactly as shown in the travel document.

If the document contains multiple given names, enter all of them in the same format as shown in the document.

If no given name is separately indicated in the travel document, enter the same name as in Person family name DE-035-02.

Person family name DE-035-02

Enter the person's family name exactly as shown in the travel document.

If the family name contains a prefix (for example, von Berg or de la Cruz), enter the full family name including the prefix. Do not omit the prefix.

If no family name is separately indicated in the travel document, enter the same name as in Person given name DE-035-03.

Date and time of embarkation DE-035-10

Enter the date and time when the person boarded the vessel.

Use the time at the vessel's port of departure or the time zone used in the notification.

Person date of birth DE-035-05

Enter the person's date of birth as shown in the travel document.

If the date of birth is not fully specified in the travel document (for example, only the birth year is available), provide the date as accurately as possible and use 01 for unknown parts, for example 01.01.1980. In such cases, add an explanation in Remark DE-003-01 indicating that the date of birth is incomplete.

Nationality

Provide the person's nationality information in this section. If the person has multiple nationalities, report each nationality as a separate entry. List first the nationality corresponding to the travel document used by the crew member for this port call.

Person nationality DE-035-04

Select the person's nationality as indicated in the travel document.

If the crew member is stateless or travelling with a special machine-readable travel document, use the following special codes defined in ISO/IEC 7501-1:

  • XXA – Stateless
  • XXB – Refugee (1951 Geneva Convention)
  • XXC – Refugee (other than Geneva Convention)
  • XXX – Nationality unspecified

Port of embarkation DE-035-09

Select the port (UN/LOCODE) where the person boarded the vessel.

Health status reported indicator DE-037-03

Select Yes if the person's health status has been reported to the port health authority.

Health status DE-037-04

Select the person's health status.

Available values:

  • R – Recovered
  • I – Still ill
  • D – Deceased

Case Disposition DE-037-05

Select whether the person remains on board, has been evacuated, or has been buried at sea.

Available values:

  • S – Still on board
  • E – Evacuated
  • B – Buried at sea

Illness DE-037-01

Provide a general description of the disease or symptoms.

Treatment DE-037-08

Provide a description of medication or other treatment administered to the patient.

Person health details comments DE-037-09

Provide comments concerning the individual case in the attachment to the Maritime Declaration of Health.

Symptoms onset date DE-037-02

Enter the date on which the symptoms began.

Place of evacuation

Complete this section if Case Disposition DE-037-05 is set to E – Evacuated.

Location of evacuation DE-037-06

Select the port or airport to which the person was evacuated.

If the location cannot be identified by a code alone, or if the code is insufficient to uniquely identify the location, also complete Location of evacuation name DE-037-07.
